Documentation that starts with the conversation
PatientsHeard records a session with everyone's consent and drafts the note. You review, edit and sign it. It documents what was said; it doesn't make clinical decisions.
The workflow
Step 01: Consent
Before recording, you confirm in the app that the patient and everyone else who may speak has agreed. Recording can't start until you do.
Step 02: Record
Pause and Stop are always on screen. If someone new joins, the app asks you to confirm their consent. If anyone says no, write a manual note instead.
Step 03: Review
The draft is labelled “AI-assisted draft — not reviewed” until you sign it. Items to check against the conversation are marked with an icon and a label.
Step 04: Show source
Each sentence in the draft links to the part of the transcript it came from, so you can check the words against what was said.
Step 05: Sign & export
Nothing leaves the app until you sign. Where your organisation requires it, a second clinician co-signs. Then copy, download or print the note.

For
Doctors
Consult notes and letters, drafted from the conversation for you to review and sign.
- Consult notes, referral letters and discharge summaries from built-in templates.
- Templates only structure what was said. Sections nobody discussed say “Not discussed”.
- Telehealth sessions start with a spoken consent script.

For
Nurses
Nursing notes and staff-only handover, with consent from everyone who speaks.
- Nursing progress notes drafted for you to review and sign.
- Staff-only shift handover in a structured format.
- Multi-speaker consent for handovers and family meetings.

For
Hospitals & clinic groups
Settings, access and oversight for organisations with many sites and teams.
- Organisation, site and department settings managed by your administrators.
- Role-based access and co-sign rules set by your organisation.
- An audit log of access to clinical content.
- Clinics can switch the scribe off by default for sensitive consults.
What it doesn't do
- It doesn't diagnose, suggest treatment or triage. The note records what the clinician said.
- It doesn't add things nobody said.
- It doesn't send or export anything until a clinician signs.
- It doesn't use patient information to train AI.
